Approved by Georgia Department of Community Health, the Georgia Pediatric Program (GAPP) is a Medicaid program that provides services to medically fragile children, under 21 years of age, in need of daily living assistance and skilled nursing care at NO COST to the family.

United Love Healthcare is a certified Georgia Pediatric Provider (GAPP) provider. “We are committed to improving the lives of children. United Love Healthcare will handle the entire GAPP Approval process to get you approved efficiently and stress free!


GAPP Approval Process

  1. Required information is gathered and consent paperwork is signed.
  1. United Love Healthcare submits all paperwork to GAPP (typically takes up to 30 days to approve)
  1. United Love Healthcare will receive a decision from GAPP and contact you to schedule the Start of Care, or to discuss the appeal process.
  1. Once approved, your child may receive up to 40 hours of weekly care.

Age Requirement

Children under 21 years old with complex medical needs requiring specialized care

Financial Eligibility

Medicaid-eligible families or those qualifying via the Family Caregiver Program waiver

Medical Necessity

Children requiring skilled nursing or personal care equivalent to what would be provided in a hospital setting

* Each child's situation is evaluated with consideration given to both medical complexity and family circumstances.

United Love Healthcare is an approved Medicaid Program under the Georgia Department of Community Health (DCH). We are apart of GAPP (Georgia's Pediatric Program) a Medicaid funded program and we specialize in skilled and unskilled private In-Home Care. This In-Home nursing support care program was designed to service eligible members based on medical necessity determination for children under the age of 20 years and 11 months.

This state-funded program was created to service children in need of assistance with both skilled and unskilled needs. Stay ahead of health concerns with certainty. We ensure timely immunizations and health screenings as scheduled. Along with personalized wellness plans designed to keep your family the healthiest.

Advanced medical care including IV therapy, complex wound care, tracheostomy management, ventilator support, and specialized feeding assistance, plus more . . .

Personal Care

Essential daily support including bathing, feeding, mobility assistance, wheel chair transfers and medication administration tailored to each child's needs

Continuous tracking of vital signs and management of chronic or complex conditions to prevent complications and hospitalizations

How to Get Started: Three Easy Steps to Join

Fill out the below form. This initiates your consultation with our team to discuss how your child may be eligible for No Out-of-Pocket Cost in-home private care. Our team specializes in getting the required Medicaid approval within 30 days.

Initial Screening


Schedule your complimentary GAPP eligibility screening and initial health consultation. Our team is eager and ready to be at your service. We'll help determine what programs and services you qualify for. After documents are collected and completed, United Love Healthcare will submit your application for approval.​​


Approval & Onboarding


Once Medicaid funding is approved based on your child's medical necessities, we will then set up an appointment for you to meet your new care team!

Here’s what you’ll typically need:

  • Your child’s Medicaid ID number
    (If your child isn’t on Medicaid yet, you may qualify through the Katie Beckett Waiver)
  • Recent medical records showing your child’s diagnosis
  • A Letter of Medical Necessity (LMN) from your child’s physician
  • Contact information of the primary care doctor or specialist for your child.
  • We assist you through the complete process. ~ No Worries! 👍

*Child must be under the age of 21 and have a medical necessity i.e. cerebral palsy, spina bifida, paraplegia, etc.
